In 2050:
health is fully understood by physical and mental wellbeing, strong relationships and a sense of purpose
- healthy choices are the easy choices, built into the way our neighbourhoods, schools and workplaces are designed
- no one goes without food, warmth or a safe place to live. Those who experience poverty receive early targeted help that is tailored to their circumstances to help support them out of poverty
- nobody faces isolation or loneliness without support around them
- when people need help, services work together around them rather than around organisational boundaries. They act early and build on people’s strengths and the strength of their communities
- the root causes of poor health are tackled
Where we're heading
- A new Dementia Hub
- Unpaid carers recognised as partners in care
- Longer, healthier lives, with the gaps between neighbourhoods narrowing
