Transition meetings
These meetings usually take place in the summer term and involve parents, professionals and settings involved in the transition.
During the meetings, we’ll:
- plan the level of support, what professionals will be involved, support to build independence and set targets for a successful transition to primary school
- agree an Action Plan covering which professionals will provide support. For example, creating a photo book of the new setting to act as a Social Story for the child
- arrange dates for meetings between settings so staff can get to know your child
- share any records that will support the child and staff understanding their needs such as a ‘Profile’
- identify which services should be involved and who the Lead Professional will be going forward
