About support to transition to primary school

We work closely with schools to support children with special educational needs and disabilities (SEND) who are starting school or moving to a new school stage. 

This is also called transition support. 

Transition arrangements

Each school has a Local Offer and SEND information report. These detail the processes for transition and how they support children with special needs. 

Special schools and schools with local authority resource provisions (LARPs) usually have their own processes. These may include:  

  • a visit where an adult will support your child to visit their new school
  • staff visits between schools
  • resources to help prepare your child for the changes
  • transition passports or profiles for your child. These help the new school staff understand their needs
