Assessments, support plans and advice
Our specialist social work team support children and young people aged 0 to 25 who have complex disabilities or have an EHCP and are in need of care and support as an adult.
We:
- carry out child and family assessments
- carry out moving to adulthood needs assessments
- write personalised support plans that address the child or young person's needs and outcomes
- offer advice, support and referrals to other services

Occupational therapy
We work with children aged 0 to 18 years who have a permanent disability and difficulty completing day-to-day tasks at home and in the community.
Our qualified occupational therapists will make recommendations to:
- promote the child’s independence
- ensure the safety of the child and the people who care for them
Recommendations focus on:
- changes that can be made at home
- equipment and adaptations
