Ask for a care assessment
To ask for a care assessment, you should first find out if we can help arrange your care.
This will tell you:
- if you're likely to be eligible for our help
- how to request a care assessment if you need one
- where to find other services and support that could help you
If you cannot use this service, contact the Adult Social Care team. They will help you get the support you need.
You can also ask a health or care professional, like your GP, to refer you to us.
If you need an interpreter to help with your assessment, you can use our interpreting service.
