Tell us your Post-16 setting preferences for a young person that has an EHCP
If a young person has an EHCP and is moving into Post-16 education or training, you will need to tell us your preferred settings.
You must select at least 3 Post-16 preferences.
The young person must not be older than 17 years by July 31 at the time of completion of this form.
Before you start
You will be asked about:
- the young person and who is completing the form
- the young person’s current school, if any
- your preferred Post-16 settings
- any other information that may help us understand the young person’s needs
Make sure you have:
- read the admission guide and criteria of the Post-16 setting you want to apply to
- consent from the young person, parent or legal guardian if you are filling this form on their behalf
If you are completing the form on behalf of the young person, answer the questions as if you are the young person.
Completing this form means you agree to the young person's preferences being discussed at a specialist placement panel if needed.
What happens next
Once you have told us your preferences, we will review your application and contact you if we need more details.
If you chose a Post-16 setting outside Royal Greenwich, we will inform the local council about your application.

You will need to complete the separate travel assistance form if the young person needs travel support. Applications are assessed against the Royal Borough of Greenwich Transport Policy.
