Apply for a disabled persons Freedom Pass
To apply for a disabled persons Freedom Pass, you must fill out our online form.
You are not guaranteed a Freedom Pass, we will use this form to check if you are eligible.
It can take up to 12 weeks to receive your Freedom Pass. Make sure you apply with enough time before your current pass expires.
Before you start
We will ask you to upload:
- a photo of yourself to be printed on your Freedom Pass
- proof of your name and age, for example a valid passport or driving licence
- proof of your address, for example your current council tax bill or a utility bill from the last 12 months
- proof of your disability or medical condition, for example diagnosis letters or medical assessment reports from the last 2 years
If you have been refused a driving licence for medical reasons, you must upload a fitness to drive form. This must be completed by a medical professional, such as your GP.
