Apply for a disabled person's Freedom Pass
You can apply for a disabled person's Freedom Pass on the London Councils website.
You are not guaranteed a Freedom Pass, we will use this form to check if you are eligible.
It can take up to 12 weeks to receive your Freedom Pass. Make sure you apply with enough time before your current pass expires.
If you have been refused a driving licence for medical reasons, we'll send you a medical fitness to drive form. This must be completed by a medical professional, such as your GP.
