What you need to apply
You will be asked to upload the following evidence on the online application form.
Proof of your name and age
We need one of these. It can be a photocopy of your:
- valid passport
- biometric residence permit - a copy of both sides
- valid driving licence
- birth or adoption certificate if your name has not changed
- marriage certificate
- European ID card
Proof of address
We need one of these. It can be a copy of:
- your current council tax bill
- your tenancy agreement
- your current TV Licence
- your driving licence
- a utility bill or bank statement dated within the last 12 months
- a rent statement dated within the last 12 months
- a benefit award letter dated within the last 12 months
Proof of disability or medical condition
This can include copies of:
- diagnosis letters
- medical assessment reports, for example, an audiologist's report
- a letter from your GP describing how the diagnosis affects you
- hospital consultant letter
- Certificate of Visual Impairment (CVI) - if you're blind or partially sighted
We may also ask you about:
- how your disability or condition affects you
- surgery or treatment you've had
If you have been refused a driving licence for medical reasons, you must upload a medical fitness to drive form. This must be completed by a medical professional, such as your GP.
