What you need to apply

You'll be asked to upload some documents when you apply. You can also send these to us by post or email if you prefer. 

A digital photo of yourself 

This must show your head and shoulders.  

You can take the photo with a mobile phone, but make sure: 

  • it's a clear and true likeness of you - you must not use any filters 
  • it's a colour photo 
  • nobody else is in it 

Proof of your name and age 

We need one of these. It can be a photocopy of your: 

  • valid passport 
  • biometric residence permit -- a copy of both sides 
  • valid driving licence 
  • birth or adoption certificate if your name has not changed 
  • marriage certificate 
  • European ID card 

Proof of address 

We need one of these. It can be a copy of:  

  • your current council tax bill  
  • your tenancy agreement  
  • your current TV Licence  
  • your valid driving licence  
  • a utility bill or bank statement dated within the last 12 months 
  • a rent statement dated within the last 12 months 
  • a benefit award letter dated within the last 12 months 

Proof of benefits you get 

We need a copy of all pages of your benefit statements or award letters. 

For PIP we need all pages of your statement including the end date and points given in each category. 

For DLA, the letter must be dated within the last 12 months. 

Proof of disability or medical condition 

This can include copies of: 

  • diagnosis letters 
  • medical assessment reports 
  • letter from your GP describing how the diagnosis affects you 
  • hospital consultant letter or report 
  • Certificate of Visual Impairment (CVI) - if you're blind 
  • SR1 or DS1500 form if you have a terminal illness 

We'll also ask you about: 

  • how your disability or condition affects you 
  • medication you take 
  • surgery or treatment you've had
