Use this service to apply for a medical assessment if your health or disability is affected by your current home.
A medical assessment helps us decide whether you should receive medical priority on the housing register.
You must already be on the housing register before you apply for assessment.
Before you start
To complete this form, you'll need:
- your housing register number
- details about your medical condition or disability
- details of your GP and any healthcare professionals supporting you
- information about your current home
- details of any disability-related benefits you receive
You will need to upload supporting documents like:
- GP or consultant letters
- hospital reports
- occupational therapy assessments
- care plans
- disability benefit award letters
If you're applying on behalf of someone else, you'll also need their consent. You'll be asked to upload a signed consent letter during your application.
