Use this form to refer a young person to get support with drugs and alcohol.
If you are a young person and want to refer yourself, use the self-referral form instead.
The person you are referring must be aged:
- 10 to 18 years old
- up to 21 years old if they have spent time living in care but are leaving (a care leaver)
- up to 25 years old if they have a statement of educational need
Before you start
You must have the young person's consent before referring them. We will not progress the referral without their consent.
We will ask you for:
- the young person's contact details
- their parent, guardian or carer's contact details
- details of other services involved with the young person
- the young person's expectations from this referral
- details about the young person's living situation, drug use and health
We will keep your responses confidential.
