Use this form if you are a young person who wants support with drugs or alcohol.
You must be aged:
- 10 to 18 years old
- up to 21 years old if you have spent time living in care but are leaving (a care leaver)
- up to 25 years old if you have a statement of educational need
If you are referring a young person, use the refer a young person for support with substance misuse form instead.
Before you start
We will ask you for:
- your contact details
- your parent, guardian or carer's contact details (if you are happy to give them)
- details of other services involved you are involved with
- your expectations of the substance misuse service
- details of your living situation, drug use and health
We will keep your responses confidential.
