Outcome 1: Residents live longer, more independent, healthier lives
We will:
- support residents to live longer, healthier and more independent lives, from a good start in childhood to ageing well in later life
- promote wellbeing across our communities and narrow the unfair gaps in health between them
- help more residents to be active, eat well and enjoy good mental and physical health
- act earlier to prevent, reduce and delay the need for care and support
- work as one team in our neighbourhoods to join up the NHS, social care and community around people’s lives and shift more care closer to home
- intervene sooner to keep people well, ease pressure on hospitals, and support unpaid carers
Our social care and independent living commitments
We commit to:
- a new Dementia Hub to help residents and families with the challenges of a dementia diagnosis
- give residents tailored support through technology such as voice assistants, video doorbells, smart home monitoring, and remote health monitoring
- helping people stay in their own home wherever possible with support that responds quickly in a crisis, supports people to return home after hospital, assess people's needs at home not on a ward.
- making it easier to live independently with:
- more specialist homes
- new models of care
- more supported employment for people with health conditions and disabilities
- more options like direct payments and individual service funds
- fewer barriers to getting around including better information about facilities so our town centres, parks and public services are more accessible
- better support for carers by identifying them earlier and making it easier to get assessments, breaks, financial advice and practical support to look after their own health and wellbeing
- promote and grow our Shared Lives programme so people can live with, or alongside, a host family
Our public health and equity commitments
We commit to:
- developing our health checks outreach initiative to help people stay well
- working with communities to close the gap in health between different parts of the borough
