Who should use this form
You may be eligible for medical priority on the housing register if your health or disability is made worse by the design or layout of your home. For example, if it’s hard to move around safely or access rooms.
If you have a medical condition or disability but the design or layout of your home is not affecting this, you can use our Housing Support Finder to explore other options.
Do not use this form if:
- your health is being affect by damp, mould or poor property conditions
- your home is overcrowded
