What you need to apply
Send copies of these documents with your application form.
Proof of your name and age
We need one of these. It can be a photocopy of your:
- valid passport
- biometric residence permit - a copy of both sides
- valid driving licence
- birth or adoption certificate if your name has not changed
- marriage certificate
- European ID card
Proof of address
We need a copy of your:
- vehicle log book (V5C), unless you have a Motability vehicle
- car insurance documents
We also need one of these. It can be a copy of:
- your current council tax bill
- your tenancy agreement
- your current TV Licence
- your valid driving licence
- a utility bill, or bank statement dated within the last 12 months
- a benefit award letter dated within the last 12 months
- a rent statement dated within the last 12 months
- a pension letter dated within the last 12 months
