You need to bring:

• proof of identity showing your name and date of birth, for example, passport or birth certificate
• your civil partnership certificate
• proof of residence.

You can provide any of the following as evidence of proof of residence:

• a utility bill dated no more than three months before the date on which the conversion is taking place
• a bank or building society statement or passbook dated no more than one month before the date the conversion is taking place
• a council tax bill dated no more than one year before the date on which the conversion is taking place
• a mortgage statement dated no more than one year before the date on which the conversion is taking place
• a current residential tenancy agreement and a recent rent receipt
• a valid driving licence.
