The more information you can give us, the quicker we'll be able to respond. We won't be able to answer your request until we've received:

• proof of identity - passport or driving licence
• proof of address.

If you're requesting the information on behalf of someone else, you'll also need to provide proof of your identity and proof of your authority to act on their behalf.
