Freedom Pass for disabled people
Complete the application form and return it to the address given on the form with:

If you think you qualify under the secondary criteria, you may be asked to hand in a form for your GP to complete (with stamp and signature).
Help with completing the forms
If you have trouble downloading or printing the forms, or need help completing them, contact the Mobility Team or visit one of our service centres, The Woolwich Centre or The Eltham Centre.

Disabled and under 18?
If you are under 18 and disabled, it is better for you to apply for a Freedom Pass than to use free travel for under-18s. A Freedom Pass will allow you to travel for free on trains. Free travel for under-18s who are not Freedom Pass holders does not extend to trains.
What happens next
If your application is approved, we will issue a letter of authorisation. Take this letter, together with one passport photograph, to the post office to order your Freedom Pass.
The post office will issue you with a receipt.
With the reference number on your receipt, you can:
- track your Freedom Pass application online
- call London Councils on 0845 275 7054 to track your application.
Your Freedom Pass will normally be delivered to your home within ten working days.
