Adult care assessment
The assessment process for adult care will reveal whether you are eligible for social care, whether you can become more independent with the right support, and what kind of support, if any, you may need in the longer term.
Throughout the assessment process, staff at the directorate for Adults and Older People will:
- treat you with honesty and respect
- work with you and involve you in making any decisions
- treat you fairly on the basis of your needs.
Initial contact
You may contact Adults and Older People's Services yourself or someone acting on your behalf may do. Some examples of people who may act on your behalf are:
- your GP
- sheltered housing scheme manager
- friend or family member
- hospital ward staff.
Some initial details will be taken and passed onto the relevant team to continue the assessment process if it seems likely that you are eligible for social care.
