Support planning

What is a support plan?

A support plan helps you tell us what changes you want to make in your life and how you plan to use your personal budget to make those changes happen. Your support plan:

  • helps you identify what is important to you and what your goals are
  • tells us how your going to achieve those goals.

You will complete your support plan once you have been assessed by Health and Adult Services and have been allocated an indicative personal budget. 

Help with your support plan 

We can arrange for support to help you complete your support plan, or you can complete it yourself if you would prefer.

Your support plan should set out how the services are going to meet your needs and outcomes identified in your assessment.

Once your support plan has been completed you assessing officer must see the plan and agree it with you.
